- In conjunction
with the Vermont Health Care Association and the state Survey and
Certification Agency, the Northeast Health Care Quality Foundation,
the QIO for Vermont, worked with 34 nursing homes throughout the
state to reduce the use of restraints by 35% and pain scores by
54%.

The QIO
for Vermont, Northeast Health Care Quality Foundation, provided
intensive consultative support to physicians to improve diabetes
care throughout the state. The in-person, telephonic, and Web assistance
provided tools and techniques to redesign care provided to patients
with diabetes, resulting in 34% improvement in performance across
the state.

The Northeast
Health Care Quality Foundation, the QIO for Vermont, worked with
the Vermont Home Care Association and the state Survey and Certification
Agency to help agencies throughout the state improve. After collaborating
with the QIO, 50% of the agencies in the state showed improvement
compared to a national average of 38% improvement. Vermont was
also one of only three states with 100% participation in the Outcomes
Based Quality Improvement (OBQI) initiative conducted from 2002
to 2005.
- Working
with the Vermont Association of Hospitals and Health Systems, the
Northeast Health Care Quality Foundation, the QIO for Vermont, assisted
all 14 hospitals in the state with a quality improvement effort to
improve the care of the most common diagnoses for patients. At the
state level, care for acute myocardial infarction improved by 79%,
heart failure improved 21% and pneumonia improved 54%.
