Working
with the Foundation for Healthy Communities of New Hampshire, the
Northeast Health Care Quality Foundation, the state’s QIO,
collaborated with all 26 hospitals in the state to improve care for
the most common diagnoses that hospitals treat. At the state level,
care for acute myocardial infarction improved 69 percent, heart failure
improved 16 percent, and pneumonia improved 51 percent.

Northeast
Health Care Quality Foundation, the QIO for New Hampshire, worked closely
with the New Hampshire Health Care Association and the New Hampshire
State Survey and Certification Agency to assist 69 nursing homes reduce
the incidence of pain by 75 percent and restraint scores by 35 percent.
- The Northeast Health Care Quality Foundation, the QIO for New Hampshire
, provided intensive consultative support to physicians to improve the
care of diabetic patients throughout the state. After collaborating with
the QIO, doctors in the state improved care provided to patients with
diabetes by 87 percent.
