Leesburg
Regional participated with FMQAI in their Surgical Infection Prevention
Collaborative. They also developed their own program known as the SWAT
program. SWAT stands for Success With Accountability Team. Members
include CVICU Director, Cardiac Medical Director, Open Heart Resource
Nurse, RT Director. Case Managers, ER Director, Quality Director and
the Quality Nurse Abstractor. The team began the process of concurrent
chart review. Then they had weekly reports of the findings to know
where they were not doing well. They began to see improvement slowly.
FMQAI recognized them as a Best Practice SIP Hospital and they shared
their results by webinar to other Florida hospitals.
The Collaborative
was led by FMQAI, the Medicare Quality Improvement Organization for
Florida, under contract with the Centers for Medicare & Medicaid Services (CMS), an agency of the U.S. Department of Health and Human Services. FMQAI is continuing to work with hospitals statewide to improve quality of care and to continue to decrease their rates of surgical infection.
