Registration
for this event will be conducted online. This simple process will allow
you to enter your meeting and special event selections, pay by credit
card, and receive a confirmation without the hassle of faxing or mailing.

For those organizations sending large delegations and/or paying with
a company check, registration can be completed using the paper form,
available by contacting Amanda Scott at ascott@ahqa.org.
Payment
Registration
forms accompanied by full payment (check or credit card) must be received
by the posted deadlines in order to receive the advertised prices.
All registrations received after the posted dates will be processed
at the current rate. Please note: Registrations without full payment
will not be honored.
Cancellations & Refunds
Cancelled
registrants will be refunded all fees minus a 20% processing fee until
November 1, 2005. After that date, no refunds will be issued for any
reason.
Substitutions
Registrants
who are unable to attend may send a substitute at no charge. All substitutions
must be received by November 17, 2005 . After that date substitutions
will not be accepted.
All
cancellation and substitution requests must be submitted in writing
to Amanda Scott at ascott@ahqa.org.

Needs
Meeting
registrants with disabilities, dietary restrictions, or other special
needs should contact Jacqueline Osborne atjosborne@ahqa.org.
