United Airlines is the official airline of the AHQA 2005 Annual Meeting. If you or your travel agent calls United's toll-free number (1-800-521-4041) to book your reservations, you will receive a 5% discount off the lowest applicable discount fare, including First Class, or a 10% discount off mid-week coach fares, purchased 7 days in advance. An additional 5% discount will apply when tickets are purchased at least 30 days in advance of your travel date. Discounts also apply on United Express, Ted and United code share flights (UA*) operated by US Airways, US Airways Express and Air Canada. Call United's Specialized meeting Reservations Center at 1-800-521-4041 to obtain the best fares and schedule information. Make sure you refer to Meeting ID Number 516SB. Dedicated reservationists are on duty 7 days a week from 8:00 AM to 10:00 PM EST.

US Airways Airlines
US Airways Airline is the official airline of the AHQA 2005 Annual Meeting. If you or your travel agent calls US Airways toll-free number (1-877-874-7687) to book your reservations, you will receive a 5% discount off the lowest applicable discount fare, including First/Envoy Class, or a 10% off with 60-day advance reservations/ticketing or per fare rule whichever is more restrictive. Call US Airways Specialized meeting Reservations Center at 1-877-874-7687 to obtain the best fares and schedule information. Make sure you refer to Gold File Number 30143148.

Rental Car
Avis Rent A Car is the official car rental for the American Health Quality Association's, 2005 Annual Meeting. The Avis Worldwide Discount (AWD) number is T653399 and the reservation number is 800-331-1600.

Room Rates

THE ROOM RATE IS $165 SINGLE/DOUBLE. Reservations may be made by calling the hotel at (415) 788-1234 or (800) 233-1234. The hotel is unable to adjust rates after check-in and cut-off, after which, reservations can only be made based on availability. Reservation cut-off is Friday, January 28, 2005. The Hyatt Regency San Francisco requires a deposit in the amount of one night’s room and tax for each of the confirmed rooms from individuals attending the meeting 14 days after the room reservation is confirmed by the hotel, but no later than the cut-off date. Personal checks, money orders, or a valid credit card with expiration date will be needed for the deposit. Cancellation for individual reservations is 72 hours prior to arrival.

Hotel check-in time is 3:00 p.m. Checkout time is 12:00 p.m. Guests arriving before 3:00 p.m. will be accommodated on a first-come basis pending availability of rooms.

In order to accommodate increased demand for additional hotel rooms during the Annual Meeting & Technical Conference we have made arrangements for a small block of additional rooms for conference attendees at the Grand Hyatt.

The Grand Hyatt is four (4) minutes away from the Hyatt Regency San Francisco, which is approximately two (2) blocks. The Grand Hyatt is in the heart of downtown, towering 36 stories above newly renovated historic Union Square, Grand Hyatt San Francisco hotel is nestled amidst premier shopping (Armani, Saks, Macy's, Levis, and Neiman Marcus - peruse the unique items at Disney, Williams Sonoma, and Niketown).

THE ROOM RATE WILL BE $165 SINGLE/DOUBLE. Reservations may be made by calling the Grand Hyatt directly at 800-233-1234 or 415-398-1234. Reservation cut-off is December 28, 2004.

In order to receive the special group room rate, please mention the American Health Quality Association. We have been advised by the hotel that attendees should make their reservations as soon as possible. Individual hotel reservations MUST be cancelled 72 hours in advance of arrival to avoid any penalties. When guest rooms are cancelled in a timely manner, a reservation cancellation number should be obtained from the Grand Hyatt for your protection.

This is a standard cancellation policy developed and enforced by the hotel not AHQA. We encourage you to request and maintain hard copy confirmation of both reservations and cancellations for your protection.

The hotel's check-in time is 3:00 p.m. Checkout time is 12:00 p.m. Guests arriving before 3:00 p.m. will be accommodated on a first-come first-served basis pending availability of rooms.

Driving from the Airport
From South Bay or San Francisco International Airport (14 miles)
Take Hwy 101 North, follow signs to Bay Bridge; Before you cross the Bay Bridge, Take 7th Street Exit; Turn left on 7th Street; Turn right on Market Street; Continue 1 mile and turn left on Drumm Street; Hotel is on the corner of Market Street and Drumm Street

Or

Take Hwy 280 North until it turns into King Street; Continue on King Street; and turn left at 3rd Street; Continue on 3rd Street; Across Market Street; where 3rd Street; Turns into Kearny Street; Turn right on California Street; Turn left on Drumm Street; Hotel is on the corner of Market Street and Drumm Street

From East Bay or Oakland International Airport (19 miles):
Take Hwy 80 across the Bay bridge ($2 toll); Take the Fremont Street Ext. Follow Fremont Street; Across Market Street; where Fremont Street turns into Front Street; Turn right on California Street; California Street will come to a dead end at Drumm Street; Hotel is on the corner of Market Street and Drumm Street

From North Bay:
Take hwy 101 south across the golden gate bridge ($5 toll); Follow the signs to Lombard Street; Take Lombard Street; To Van Ness Avenue; and Turn right; Continue one mile on Van Ness and Turn Left on Clay Street; Clay Street will come dead end at Drumm Street, where you will turn right and continue one block to the hotel; Hotel is on the corner of Market Street and Drumm Street

Dress Code
AHQA has designated the dress code for the 2005 Annual Meeting & Technical Conference as casual sportswear.

Weather
February temperatures average around 70 degrees (average high) and low around 45 degrees (average low). Visitors are most comfortable with a light jacket or coat. An all-weather coat will take the chill off in the evenings, as well as lightweight fall clothes are the suggested dress during the day. Please be aware that indoor meeting room temperatures tend to be cooler, so dress accordingly.
