for all medically underserved people with chronic diseases. The Health Disparities
Collaboratives were developed to change primary health care practices in order

Institute
for Family Centered Care provides essential leadership to advance the
understanding and practice of family-centered care. By promoting collaborative,
empowering relationships between providers and consumers, the Institute facilitates
family-centered change in all settings where individuals and families receive
care and support.
Institute for Safe Medication
Practices works closely with healthcare practitioners and institutions,
regulatory agencies, professional organizations and the pharmaceutical industry
to provide education about adverse drug events and their prevention. The Institute
provides an independent review of medication errors that have been voluntarily
submitted by practitioners to a national Medication Errors Reporting Program (MERP)
operated by the United States Pharmacopeia (USP) in the USA.
Midwest
Business Group on Health is a coalition of employers working together
to provide leadership and knowledge to continuously improve the quality and cost-effectiveness

Best Practice Network is a not-for-profit entity whose vision is to provide

National Coordinating Council for Medication Error Reporting and Prevention
intends to mount a nationwide campaign for medication error reporting and prevention

National Quality Forum is a not-for-profit membership organization created
to develop and implement a national strategy for health care quality measurement
and reporting. A shared sense of urgency about the impact of health care quality
on patient outcomes, workforce productivity, and health care costs prompted leaders
in the public and private sectors to create the National Quality Forum as a mechanism
to bring about national change.
The
National Safety Council is a nonprofit, non-governmental, international
public service organization dedicated to improving the safety, health and environmental
well-being of all people.
The
Quality Indicator Project began in 1985 as a voluntary pilot project of
seven Maryland acute care hospitals that wanted to test a handful of clinical
performance measures for data collectability and usefulness in quality improvement
activities. Today, the Project has more than 1,800 participants across four different
indicator sets: acute care, psychiatric care, long-term care and home care.
The
Quality Interagency Coordination Task Force (QuIC) works to ensure that
all Federal agencies that purchase, provide, study, or regulate health care services
are working in a coordinated way toward the common goal of improving the quality
of care. The QuIC seeks to provide information to help people make choices, to
improve the care purchased and delivered by the Government, and to develop the
infrastructure needed to improve the health care system.
The
SCRIPT Project works to improve medication management. Public and private
sector groups were organized in 1997 to form project, initiated and currently
funded by HCFA. The expected outcome of the project is the development of a core
set of outpatient medication measures that can be used in a variety of settings.
